Pilot shortened the lines attached to the speed bar, this caused the speedbar to be applied during weight shift turns as the extra weight on the front of the harness seat would press on the speed bar.

During light wind forward launch pilot appeared to encounter sinking air. The student continued to run, but stepped onto uneven ground resulting in a broken ankle.

Student forward launched assymetrically which caused her to fall over.

During an attempted forward launch the canopy inflated slightly off line, the student continued with the launch but soon realised that she was heading for a tree, she applied full brake - the wing overshot and draped itself over a tree.

Pilot adjusted the length of the speedbar to make it easier to use. Noticed that during weight shift turns the speed system was unintentionally being applied causing small collapses.

Reserve parachute fell out of the students harness during an attempted forward launch.

Whilst ground handling a new canopy the pilot noticed that the Brummel hook connection to the speedbar had become detached. On one occasion the left control line became caught in the slot in the brummel hook.

Whilst soaring in windy conditions with the speed bar applied the canopy suffered a 80% deflation causing the glider to turn right. The situation deteriorated with the canopy cravtting and then entering a full stall. Pilot impacted heavily.

Severe bruising to back, ribs and knees.
