During repack it was discovered that pilot had connected maillon at end of chute bridle to second handle on deployment bag as well as to harness bridles. Impossible to deploy as result.

Pilot turned wrong way on forward launch causing risers to be twisted. Pilot was lifted off ground still twisted and was seen to pull one of the controls in panic. Glider turned sharply throwing pilot against a boulder with great force.
