During a deployment and repack practise the pilot found that the emergency parachute would not deploy. It had been packed in the harness incorrectly. Parachute was then correctly fitted by a qualified packer and functioned correctly.

After two failed launches the pilot did not realise he had turned to face the glider in the opposite way to his normal routine. On launching the third time the pilot found he had a full twist in his risers. The pilot was lifted by a gust and crashed sideways.
