During repack and annual inspection (with a licenced packer) it was noted that the rubber bands used last year had degraded, broken and stuck to some of the lines on the reserve canopy causing staining in some areas. The licenced packer stated he had never seen this kind of deterioration before.

Pilot was too slow on approach causing the glider to drop a wing. Pilot lost control of the glider while attempting to correct the turn. Landed heavily.

Pilot was distracted from approach plan by others shouting. Eventually realised they were shouting about a spot landing competition. The pilot made a tighter than anticipated low turn in an attempt to hit the spot and landed heavily.

Pre-flight check revealed the outer sheath of the slider on the harness had worn away, and broken away, leaving just the inner cords exposed and supporting the hanging of the harness.

Having carried out a perfect flight, the student made non-briefed, low turn on final approach in order to land "closer to the spot". Student crashed into parked car.

Pilot chose an adequate landing field but failed to see power lines on the upwind edge, turned sharply to avoid flying into the power lines and landed heavily on a turn without performing a PLF.

While holding the front wires under tension during a pre-flight check, the pilot noticed the rear wire snap at the end of the swage coming out of the keel. The pilot suspects that where the wire comes through the keel there is not enough clearance and no protection for the single wire coming out. While the glider is parked and the wind moves the glider it has slowly cut through the wire.
