During tow launch the tow line finger trap loop came loose approximately 40 ft AGL. The pilot applkied brake to control the forward surge and impacted the ground feet first.

Pilot flew canopy slowly whilst close to the ground - canopy stalled asymmetrically at 20ft - pilot impacted the gound.

Pilot started engine of paramotor whilst supporting the frame. The canopy remained packed away. The engine revs were high pushing the unit forward. The pilot reacted by pushing out with his left hand which contacted the prop causing injury.

Left hand Index finger 50% of Tip severed, fingers 2 & 3 removed amputated to first joint. Little finger 3mm of tip removed

Whilst ground handling the pilot realised that the split A risers on the right of his canopy were rigged incorrectly from the manufacturer.

Inexperienced pilot flew into the rear of another PG. His canopy suffered an asymmetric collapse followed by a 90 degree turn resulting in downwind track. The canopy dived forward and did not regain normal flight before the pilot impacted the hill.

Inexperienced pilot launched and gained height and flew into the rear of another airborne pilot - the canopy striking the airborne pilots body. The canopy of the lower pilot collapsed completey and the pilot fell 100ft to the ground.

Pilot attempting to utilise lift whilst low and in the lee of a tree line. His canopy suffered a collapse and then entered a spiral dive. The pilot impacted the slope.
