During PG tow launch the weak link broke causing the pilot to fall backwards - as he did so the quick release mechanism hit him in the mouth breaking a tooth.

Pilot misjudged altitude on downwind leg of landing approach, made final turn into wind too low and was unable to complete the turn.

Both pilot and passenger went to hospital for exrays but no reported injury

Pilot was nable to complete the final turn into wind during the landing approach due to insufficient height - landed cross wind falling onto the passenger.

During tow launch the left control line came free from the control loop. The tow was terminated at a safe height and the pilot landed maintaining direction of launch.

Inexperienced pilot was unaware that the wind had increased. Her canopy was unable to penetrate forward, so she applied speed bar. At 20ft the canopy suffered a front tuck and the pilot fell to the ground.

Pilot waited for gap in cloud before launching from mountain - following launch he decided to try and gain height. He then became engulfed in cloud, lost orientation, and flew into the mountainside.
