Pilot crashed on take-off on winch launch. Pilot called "all out" before wind was fully on anticipating it would straighten but it did not. Also, possibility of thermal turbulence sinking down from above during launch pushing right wing down.

Forward launch on winch. Wing off line to right. Pilot control input to the right causing wing to go towards lock out. Operator guillotined. Student instructed to pull left and finally instructed to PLF.

On landing approach, the experienced pilot gave way to a low airtime pilot and hit a tree to avoid mid air collision.

Pre flight inspection of Icaro helmet revealed that one of the two retaining loops of plastic coated cord had snapped and detached at the front end at it's retaining point where it is fixed with a rivet. On inspection of spare helmet, it was found that in the same place, the outer plastic coating had snapped, but was still being retained by the inner cord core.

Pilot slipped during reverse launch ground handling practice, coupled with a sudden gust of wind. Resulted in sudden lift from and immediate repatriation with the ground at speed.

When Taking off, the student, caught his hand on the reserve handle and brushed the lower bushes of the take off area. The reserve was deployed, which in turn made the main glider drive down. the student remained clam, and landed in the trees in front of the take off.

Pilot fell over his own feet after he had landed and fell on his face. His sunglasses dug into his nose which needed a few stitches.

Pilot spun glider while attempting to thermal. Pilot suspects lack of sleep and dehydration were significant factors.

Pilot turned low in a thermal close to trees. Pilot then hit sink and was caught in the rotor behind the trees. Glider suffered asymmetric collapses but pilot was able to land but hit a fence post.
